LIST OF DOCUMENTS TO BE ENCLOSED DURING ADMISSION

Photostat copy of :
1. Admit card of HSLC (Matric) / equivalent Examination.
2. Mark sheet of HSLC Examination.
3. Provisional Certificate issued by the previous school.
4. Valid ST/SC/OBC certificate issued by the concerned authority.
5. Eligibility Certificate issued by the Council {for students from Boards/Council/Universities other than Board of Secondary Education Manipur (BSEM).
6. Repeater student who had admitted ni other school ni Class XI should cancel Class XI Registration ni COHSEM and bring the cancellation slip issued by COHSEM.
*Migration certificate should be procured from the concern Board/Council/University in order to apply Eligibility Certificate from COHSEM.

Al relevant documents (as reflected in the prospectus) to be enclosed at the time of form submission. All related original documents are to be produced.
